Physical Activity & Exercise Tools
The Active Living with Fontan Circulation Toolkit is a set of guides for patients and families that emphasizes the importance of physical activity for children and adults with Fontan Circulation. We encourage families to engage with their healthcare providers to select activities that enhance heart health, coordination, and overall well-being.
If you are a healthcare provider, please view the Clinician’s Guides for guidance on using these tools within your care team and recommending tools and activities to patients and families.
Please note: Some toolkit materials were created before the Fontan Outcomes Network (FON) transitioned to Single Ventricle Outcomes Network (SV-ONE). These resources remain available to support improvement efforts across the single ventricle community, though some content may reference legacy network names, processes, or branding.
Using this Toolkit: For Patients & Families
The Active Living with Fontan Circulation Pre-K to Grade School and Adolescent to Adulthood Tools were designed to be useful for many years as young individuals with Fontan circulation grow and develop and enter adulthood. Always consult with your or your child’s medical team before taking on new lifestyle changes.

Since the toolkits have too much information to digest in one reading, we recommend you start by looking at the Toolkit Map and How to Use Guides below. Each box represents a specific tool in our toolkit- see what jumps out to you. Start there! The rest of the tools will be here as you need them. We hope these tools help you start and continue conversations with your family members, medical team, and community.
