HEALTH EQUITY IMPROVEMENT PROJECT

Brief Summary: This improvement project worked toward reducing or eliminating disparities in outcomes based on race, ethnicity, and insurance type by identifying and removing barriers to prenatal care, including prenatal diagnosis and coordinated delivery of mothers of infants with congenital heart disease.

SMART Aim: By December 2023 we aim to:

  • Decrease the gap in rates of coordinated delivery by 50%:

    • Between White and Hispanic patients

    • Between White and Black patients

    • Between White and “Patients Who Identified as Other”

  • Decrease the gap in rates of prenatal support provided between commercial insurance and government insurance from 5% to 0%

Participation Details: NPC-QIC centers were invited to participate in the ongoing monthly Health Equity webinars. At the conclusion of the webinar series, centers were invited to continue their participation through the use of QI tools to reliably test health equity interventions.
