Project Phase:

The Physical Activity and Exercise Project is in the Pilot and Spread phase.

Brief Summary:

SV-ONE has been leading a collaborative effort to promote safe, active lifestyles for individuals with Fontan circulation, with a focus on improving exercise capacity and quality of life. Over the past year, participating centers have engaged in implementing the PROMIS Physical Activity Questionnaire locally. Centers are using the Active Living with Fontan Circulation toolkit as a resource to have conversations around the PROMIS and physical activity with patients and families. They are testing new Quality Improvement tools, sharing their progress, and submitting data for the project.

SMART Aim:

Ensure that physical activity is reliably assessed and discussed at clinic visits so that >90% of routine clinic visits document a PROMIS assessment and discussion of physical activity.

Measures:

  • Percent of routine clinic visits for pediatric and adolescent patients in which a PROMIS Physical Activity assessment (by the patient or parent) is documented

  • Percent of routine clinic visits for pediatric and adolescent patients in which a PROMIS Physical Assessment is reviewed and discussed with the patient and/or family

Participation Details:

There was an initial QI cohort of 36 care centers who have been actively engaged. We are exploring different ways for centers, patients, and families to engage with this topic in the future.
